Announcement: Pagosa Medical Group is merging with Axis Health System

Eight years ago, we opened Pagosa Medical Group to respond to a variety of our community’s unmet healthcare needs. We started small, and due to the trust we have built with you as our patients over the years, we have grown into a bustling clinic with 11 providers and nearly 30 staff members.
Our success has been undeniable, and we are proud of our many accomplishments in Pagosa Springs. We established important partnerships with local specialists and have provided primary and urgent care services to expand access to care. We served as leaders through the COVID-19 pandemic with testing, vaccinations, and therapeutics.
Our success has also been our Achille’s heel. We need more space and more management infrastructure. Faced with this challenge, we sought out Axis Health System as a community partner that shares our values of putting patients first.
We are excited to share some news with you. On August 4, 2025, Pagosa Medical Group will become part of Axis Health System, a trusted provider of comprehensive healthcare in our region. This move represents our ongoing commitment to delivering exceptional healthcare while expanding the resources and services available to you and our community.
Here’s what this means for you as a valued patient:
Your providers, nurses, and office staff aren’t changing. You will continue to see the same trusted people who know you and understand your health needs.
Your care remains our priority. The same high-quality, personalized care you expect from us will continue without interruption.
You will have easier access to more care. Axis Health System provides primary care, oral health, pharmacy, psychiatry and behavioral health services which will also be available to you as our patient.
We want this transition to be smooth and we are committed to continuing to be part of your local healthcare team. We very much want to bring each and every one of you with us and we ask you to lean on that same trust you have given us over the years as we enter the next phase of our service to you.

The Pagosa Urgent Care will stay right where it is with the same services and hours of operation
that are currently available.
Yes! You will continue to see your current provider, as our team is committed to maintaining
continuity in your care. You will have access to a wider range of services, including behavioral
health, psychiatry, dental, and pharmacy, all currently provided at Axis’ Pagosa Springs Integrated
Healthcare location.
You will receive an appointment reminder that clearly confirms the location of your visit, so you’ll
know exactly where to go. Please look for this reminder or contact us if you have any questions.
You can pay your existing PMG bills on our website or by mail.
Yes, you will receive access to a new patient portal at your first appointment as an Axis patient.
No action is required on your part. Our team is actively working to ensure a smooth transition for all
of our patients. If you have any questions or concerns, please reach out to us for assistance.
Yes, your medical records will securely transfer to Axis, ensuring your provider has all the
necessary information to continue your care without interruption.
Our Services
Urgent Care
A good option for non-emergent medical issues, including minor injuries and common illnesses. We see patients on a walk-in basis, no appointment necessary.
Primary Care
As your partner in health, a primary care provider is dedicated to your well-being, focusing on preventative and continued care. Please contact our front office to schedule.
Diagnostic Imaging
X-rays are an effective way to identify a variety of health conditions and concerns. Please contact our front desk to inquire about our x-ray availability.
Lab Services
A LabCorp phlebotomist is available 8am to 4:30pm, Monday through Friday. (Closed 12PM to 1PM for lunch.) Please contact our front office to schedule.
Endoscopy + Colonoscopy
Dr. Shaeffer has screened for, diagnosed and treated more cases of colon cancer than anyone in Archuleta County. We schedule patients on a referral basis.

We’re conveniently located off of US-160/San Juan Street in Pagosa Springs, CO 81147 between the TBK bank and Roadhouse restaurant. To access our main entrance, please park in the large parking lot, closest to the highway. Look for the large, red “URGENT CARE” sign posted above double-doors. This is where you’ll check in. Our lab entrance is located on the backside of the building, closest to the restaurant, with limited parking reserved for patients with scheduled appointments. There will be signs to help direct you.

You will receive an e-statement via email and a text confirmation. Please verify and update your email address. If you prefer to opt-out of e-statements, please let our front desk or billing team know and provide your current mailing address.

You can pay your bill online (quick tip: you must include “PS-” followed by your unique 6-digit statement number). You can pay your bill over the phone, by calling (970)259-8681 or (970)372-0456. You can pay your bill by mail or pay your bill in person.

Prior to your appointment, please verify and update your insurance information. It is your responsibility to contact your insurance company for details regarding coverage. You may need to notify your insurance or request prior approval before you receive treatment at our urgent care clinic. Co-payments are due at the time of your visit. You may owe a co-insurance or deductible amount after your claim has been processed. This is a part of your contract with your insurance company.
We offer affordable cash-pay options for patients who are either uninsured, have high-deductible plans or have health-sharing plans like Medishare.
Care is not based on your ability to pay.
